I purchased a cover from Intensity sails, www.intensity sails.com, and was very happy with the price, quality and service. They charge about $240. plus shipping which varies on your location, they ship from the manufacturer in California. The cover fits around the rub rail without the mast or boom crutch in place and has tie down straps to secure it. I keep the boat on a trailer with the mast on a support bar mounted on the trailer tongue and resting on the outboard bracket at the stern. To keep water from collecting in the cover at the stern, I fabricated a support bow from a length of fiberglass flat stock purchased at West Marine. The boat is now clean and dry and the deck is protected from constant UV exposure.

If looks don't matter, the heavy duty white tarp might be the cheapest solution.

They cost between $30-40 in that size and last 2-3 seasons. I figure that by the time I've gone through six of them, I'd be past the end of the life of any custom-built cover.

The mast rests on a mast crutch in the front (inserted into the hole in the cuddy) and another one (partly visible) that's fixed where the rudder goes.

(Note the opening for ventilation at the back).

I just purchased the mooring cover from SLO Canvas for $149. Ordered it online and received it in about 3 days. Looks to be well made, but is specifically designed for use with the mast up and boom supported, whether by crutch or topping lift. Since my boat will be on a mooring ball, this was a very good solution for me.
